Make the observation record understandable
A useful report shows the question, tool, collection method and date alongside the saved answer. Mentions, citations and recommendations need distinct definitions. The denominator should make clear how many checks produced valid answers and how many failed or were unavailable.
Compare equivalent panels and conditions across periods. If a question, market or collection method changed, mark the break rather than presenting an unqualified upward or downward trend. A report is a sample of recorded answers, not a census of all user conversations.
Connect each finding to completed or proposed work
For an improvement, show its scope, publication status and verification. A third-party correction request remains a request until the external page changes and that change is checked. A completed source update and a later citation are separate observations.
End with decisions: which factual gap to resolve, which page needs clarification and what evidence is required before publishing a claim. Keep paid scope and responsibilities visible so the next month can be reviewed against concrete commitments.

A practical example
A report shows a corrected service page, its publication date and unchanged recommendation counts. That is a valid record of completed work even though a visibility increase has not yet been observed.
